Experienced HIV Clinician, Specialist in Hepatits-C and Expert in providing Medical Care to LGBTQ Youth
Kathleen Bernock

Kathleen Bernock is a primary care provider specializing in HIV care and prevention, Hepatitis-C treatment, and LGBTQ health and is the Lead Clinician for HIV Pre- and Post-Exposure Prophylaxis (PrEP & PEP) Services. She has specialty certification from both the American Academy of HIV Medicine and the Association of Nurses in AIDS Care. She also offers substance use treatment, including MAT (Medication Assisted Treatment) and has obtained a waiver to prescribe Buprenorphine for prevention of opioid overdose. Prior to working as a Nurse Practitioner, Ms. Bernock worked with LGBTQ youth as well as in homeless advocacy and asylum services for survivors of torture and gender-based violence. She firmly believes in taking a holistic approach to medicine utilizing patient centered care.

Successful Start is a FREE program that provides parents with supportive services and education relating to childbirth and parenting. This home visiting service facilitates a healthy environment for children to develop and grow... and to empower parents to become independent and self sufficient in meeting the needs of their families.

Who can apply? Families who live in Zip Code 11216 and have a child under 3 months of age.

How do I apply? Complete the form and return it to the person who gave it to you. You can also call Successful Start and speak to a Family Assessment Worker. (347) 464-6590.
