Sliding Fee Discount Program

The Bedford-Stuyvesant Family Health Center is committed to providing high-quality, affordable health care to all individuals, regardless of their ability to pay. To support this mission, we offer a Sliding Fee Discount Program designed to reduce the cost of care for patients based on household income and family size.

The program offers reduced rates for eligible patients on services such as copayments, coinsurance, deductibles, and certain services not covered by insurance. Discounts are based on the Federal Poverty Guidelines to ensure fairness.

All patients are encouraged to apply, regardless of insurance status. Eligibility is based solely on household income and family size. Those uninsured, underinsured, or with high out-of-pocket insurance costs may qualify.

Applications can be picked up at any BSFHC location or through our billing office. You'll need to provide proof of income (pay stubs, tax returns, or benefit statements). Reapplication is required annually or if your financial situation changes.

If you don't meet the criteria but still face financial hardship, our team will work with you to establish a flexible payment plan so you can continue accessing care.

No patient will be denied services due to inability to pay. Speak with a case manager or financial counselor if you’re worried about costs—we’re here to help support your health and financial well-being.
